Service Agreement
Our Services and Procedures
At Pelvic Care Physiotherapy we value clear communication. Prior to the commencement of any initial appointments we ask all our patients to sign a Service Agreement so that the terms and conditions of our service are clear. This helps us to provide an efficient, safe and quality service.
1. What should I expect from my appointment?The physiotherapist will take time to understand and assess your problem. A holistic approach is used that takes into consideration the biological problem, psychological background and social context to ensure the best outcomes are achieved. Together we will choose the best way to assess your problem. Assessment may involve an internal (either vaginal or rectal) or external (fully clothed if desired) check.
Treatments include education, exercise prescription, products (if required). The physiotherapist will bring any necessary equipment to the appointment. As a provider of healthcare in the home, the physiotherapist will comply with the highest of safety and hygiene standards. The treating physiotherapist is adequately trained in COVID-19 safety policies and infection control practices. If anyone in your home has COVID-19, please contact us on 0421 694 434 to reschedule your appointment or convert your appointment to Telehealth.
2. ReferralsA referral is not a requirement to book an appointment. Please bring any relevant letters or test results to your appointment or forward them with your online booking form.
3. PaymentWe are a private practice and you are responsible for the payment of fees for services provided by us. If you have Private Health Insurance extras cover you may receive a rebate. Speak to your insurer to confirm rebates (differ according to insurer and level of cover). Treatments will need to be paid in full on completion of your appointment and then claimed back from your insurer.
Some patients will be able to claim a Medicare rebate. Discuss with your GP if you are eligible for an Enhanced Primary Care (EPC) plan. These plans are for individuals with chronic conditions and entitle patients up to five subsidised allied health professional visits within a calendar year. These appointments require a referral from your GP and can take time to set up. They must be arranged with your GP prior to the physiotherapy appointment to be claimable. Treatments will need to be paid in full on completion of your appointment and then claimed back from Medicare. 4. TravelYou might be charged an additional fee for travel if we travel further than our serviceable catchment. These specific travel costs will be discussed with you at the time of making the appointment.
5. Cancellation PolicyPlease note that 24 hours’ notice is required to cancel an appointment to avoid incurring a $50 cancellation fee. If you fail to attend your appointment without providing any notice of cancellation, you will incur a fee equivalent to the full cost of your appointment.If you need to cancel your appointment, please contact us on 0421 694 434.
